The SessionManager could not connect to either the CCXML or the vxmlmgr process during initialization because either the CCXML, the vxmlmgr, or both processes were not running properly.

Procedure
- Verify that both the CCXML and the vxmlmgr processes are
running using one of the following methods:
Review the home page of the Media Server Service Menu.
Run the command stat.php on the Media Server.
- Restart the Media Server.
