Default Avaya 1608-I help troubleshoot

Hello,
I am new here and would like to ask for some assistant from the experience people. The phone I have is a 1608-I and when I enter an extension I created onto the phone it accepts. But at the password screen I enter no password or the "clear = 25327" code and it says "Extension Error".

I have the FileSv IP setup to the same IP from other phones, the same for Vlan ID, Vlan test and Callsv. It has an IP and I can ping it.

Also, when I go to GEDI and typed in "status station xxxx" under service state, it states "out-of-service".

Help are greatly appreciated
