Default Cannot receive voicemail messages

I have one user that cannot receive any voicemail messages. When I login to her mailbox from inside or outside, it says she has 0 messages. I've logged in to the IP Office Manager and ensured that the user's Voicemail On setting is checked and active.
I've checked Voicemail Pro Client (Intuity) and browsed to Specific Start Points > Users. I noticed that the user's Last Access has "NEVER ACCESSED" message, even though she is currently accessing it. She can send out voicemail messages but cannot receive.

Avaya IP Office 9508.
IP 500 V2
IP Office Manager 10.1 (67)

Maybe the user's voice mailbox got corrupted? If so, how to fix corrupted voice mailbox?
This error is only happening on one user.

Solution Delivered Delete and Recreate

Back when I was running Definity/Intuity, the answer was to delete the offending subscriber and recreate it.
