Default Problem with upgrade bcm400 to bcm450 r 6

Default Problem with upgrade bcm400 to bcm450 r 6

I have seen this error in past. These could be happening:
-source BCM not patched latest
-BFT connections is loose(try re fitting BFT)
-BFT is bad and needs RMA
