Default DS30B Port 6 Issue

Having a very strange issue....

On IP500 DS30 B modules - when connected to R11.1.2.1.0 Build 3 to R10 Control Unit test fine except port 6.

The powers up but the LED on port 6 goes solid, then goes out and starts to blink. This just repeats and the phone does not work. I've used multiple 1408/16's and different IPO's with R11.

Tried downgrading to R9 CU and it works fine ... back up to the R11.1 CU and port 6 always fails -

Multiple DS30B

Any ideas?

Thanks!
