Question Provisioning 11xx and 2004 using DHCP option 128

As anyone used a TFTP server with .prv files to auto configure IP phone sets based upon hardware type?

If so can you share with me;

a) What the DHCP option sting was?
b) What the naming of the .prv file was?
c) What is the format of the .prv file?

Default provisioning server

I would like to know where the docs are on how to set this stuff up are? I am having a tough time finding them.
