Default Nortel 6140 HAT Software

I'm having an issue with HAT (Handset Admin Tool) with a 6140 phone. I had it working at one point, then all of the sudden, I can't never to get the software to see the phone. I launch the software, power down the phone and place it the dual charger. The phone will then display "USB Mode ON...enumerating" and that's it. The software never shows the phone connected. I'm using HAT software version 02.08.12.00.
Any ideas as to what may be happening?
