Default AVP sends connectionDisconnect event to vxml-app but the call is active

Hi,
we are facing a scenario where
--vxml application receives connectionDisconnect from AVP from H323 port.
--but the AVP-h323 port shows as Active call even after it sends connectionDisconnect event to VXML application.
--At the time of checking what status the port was in Avaya CM report - on report Agent\Queue Status - window it shows as OTHER state.

Environment: Avaya Voice Portal 5.1,
Application: CCXML, VXML - OD 6.0
call path:
[application]---------[AVP]-------(h323)---------------[Avaya CM]--------pstn cloud

Call Scenario steps:
1.CCXML script used to call a mobile number.
2.once the call was answered, vxml application (with aesconnector) invoke to play some audio files

Note: --at this stage the H323 port having 1 active call
and the below steps followed from vxml script

3.Do hold the active call,
4.Make another call to Avaya VDN - via the same H323 port
5.From VDN the call routes to an available agent extension.
6.Once the call was answered, play some audio file

Note:--at this stage the H323 port having 1 held call & 1 active call,

7.Now, do conference the call
8.After the conference was successful and the parties are in conversation.

Question:
1. For the h323 port blocking scenario, from anywhere between point 3 to point 7 the vxml application receives ConnectionDisconnect event and vxml app thought the mobile party was dropped/hangup and do disconnect if there is any secondary active call in the same h323 port and does the cleanup and exists.

Why the AVP H323 sends the ConnectionDisconnect event if the call was active.?

2.The connectiondisconnect happens from the H323 in random intervals - and in random ports.

1.How to initiate Service Request, Can you pl share the references
2.What details you required at the time of creating service request.
3.Is there any support agreement to be signed with Avaya to work on Service Request,
