Default 1608 phone resetting to factory default

how to reset a 1608 phone back to factory default. once a 1608 has downloaded an extension from IP Office I find that the phone always remembers that extension even if you delete the extension from IP Office.

any suggestions would be helpful. Using IP Office 8.0 (53)
