Default 8612XLRS anomaly
Has anyone seen intermittent connectivity problems on 8612XLRS modules after upgrading to 5.1.5.1?

Not sure if this would apply to your situation, but I found this in the release notes

On 8612XLRS modules with DDM enabled, wait 3 minutes after module
initialization before you enter show sys pluggable-optical-modules
commands to avoid errors during the initialization.
