Scenario 1:  After upgrade from 8.1.65 to 8.1.85 phones are showing on Display "Upgrade: file access error". There is no service on the sets with this issue.

Scenario 2.  Upgraded from 9.1.0.0.437 to 9.1.800.172 Monitor trace showed it was unable to find 1400R46.bn

Scenario 3. IP Office version 9.0.4. Phone f/w: R40. When the IP Office is rebooted, the 1408 does not boot and does not update the f/w as expected.
Moving the phone to a different port on a different card did not trigger the f/w update.
The correct f/w was on the SD card.
The phone f/w was at a later version that required for the version of IP Office. IPO 9.0.4 was released with f/w version R38 but the phone was R40

Scenario 4.  Upgraded from 5 to 10.0.0.3 (steping up) 1416 getting upgrade file access error, 9508 never came back up, says please wait.

Scenario 5.  Upgraded from 9.1 to 11.0.4.0, 1408 and 1416 getting file access error
