OneX Mobile for Android on CS1K - Can't call internal extensions

I have set up an IPphone and an android with the One-x mobile lite and both are working fine. Have a new user who also has an Android (Samsung Galaxy S5) and I am having several issues. Calls are forwarding to the cell phone even though we did not turn on simultaneous ring on the cell. I have tried disabling the simultaneous ring, that does not work. In addition, I cannot call an internal ext from the one-x mobile on the cell phone. I traced the uext and the result is below. Please note that I altered the calling number and the called number to protect the innocent. Also, the android that works is using AT&T and the one that does not is using Verizon.
