9641 SIP Phone Button Assignments Not Working

I have a 9641 in a lab environment that registers as an AST device in System Manager and is getting PPM information according to the traceSM. However, I can't get any of the button assignments to work?

I've tried assigning them via System Manager and the CM. However, the labels do not show up and the buttons don't function.

Can someone help me or point me in the right direction? I just don't understand what I could be missing. Doesn't seem like it should be this hard.
