TFTP Issues

Hi all, we have an IP500 v1 running 4.2(20), 2 x DS 30 and a DS 16.

we are tying to re-configure a faulty DS16. we had done a AT-X2 and AT-X3 to erase the switch which was ok. but when trying to get the new files from the TFTP server we get "TFTP Poll Failed".

We have checked the TFTP server and we can get and place files via DOS with no issues.
we are using the seperate Avaya TFTP not the manager.

Looking at the logs on the TFTP is looks like it is trying to find a file "nasystem/who is" we have no nasystem in our working directory or any where that I can find.

The TFTP server is working for the phone upgrades etc....

Help?
