SD card failure

After licencing and initial reboot the IPoffice V2 is in a continuos reboot loop and I am unable to login via manager. DTE is providing the following errors - "Failed to read header from main image (0x00000079) and "system primary failed to open correctly". With this information is anyone able to provide feedback on what I believe to be an SD card failure

I think you are upgrading a system to 9.1 were the boot loader is not upgraded yet.
Try this: copy the file ip500v2.bin from the 9.0.x Manager to the SD card System\Primary folder and reboot the IP Office, wait untill you see the sentence "TA started" in the DTE Monitor.
Then copy the ip500v2.bin file from the 9.1 Manager to the SD card and reboot.
