DTMF breakout from voicemail problem in IPO

Having an issue where when someone connected to a user's voicemail presses 0 (or any other key with a DMTF breakout defined in User/Voicemail/DTMF Breakout/ ) instead of being transferred to the extension, they hear the name recording of that extension.

    E.g.

If User 105 has the DTMF 0 breakout field in their voicemail tab pointing at User 101,
when someone connected to User 105's voicemail presses 0, instead of being transferred to user 101, they hear user 101's name recording, then dead air, and System Status shows them as still connected to User 105's voicemail.

This happens whether the DTMF points at a user or a group. If there's no name recorded for the extension, they hear the default Avaya voice reading the number of the extension.

This is on a 500V2 running version 9.1.9.0, except for the UC voicemail module which is on version 9.1.8.0. I wonder if the sub-version mismatch could have something to do with it.

Is this a known issue? Any ideas?
