Digital phone show " installation test u25p00 ready for download"

I have 1408 suddenly don't work and show "INSTALLATION TEST U25P00 READY FOR DOWNLOAD" message on the screen. Have unplug & plug and reboot connected IP Office Rls.11,0,4. The problem still there!
